Analysis

WHILE MAKING A TURN AT THE END OF A SWATH RUN, THE PILOT HEARD A BANG AND THEN NOTICED A LOSS OF ENGINE POWER. HE ENTERED AN AUTOROTATIVE LANDING FROM A LOW ALTITUDE AT LOW AIRSPEED. SUBSEQUENTLY, THE HELICOPTER LANDED HARD AND ROLLED OVER ON ITS RIGHT SIDE. AN INVESTIGATION REVEALED THAT THE SPUR ADAPTER GEARSHAFT HAD FAILED FROM FATIGUE. THE FATIGUE INITIATED ON THE LOADED SIDE OF A SPLINE TOOTH AT AN AREA OF HEAVY WEAR. TOTAL FLIGHT TIME OF THE FAILED PART WAS 3491 HRS. THE SPUR ADAPTER COUPLER THE COMPRESSOR TO THE N1 TURBINE.
