Factual Information

On November 25, 2009, a Cessna A-188B, Ecuadorian registration HC-CEU, registered to and operated by Russagrif, impacted terrain in the Los Rios Provenance following a loss of engine power, immediately after takeoff. The pilot sustained serious injuries and the airplane was substantially damaged. The flight's last departure point and its intended destination were from an unpaved area near the city of Quevedo, Ecuador. According to information provided the airplane is equipped with a Teledyne Continental, IO-550D engine and had approximately 613.10 total hours of time in service. The investigation is under the jurisdiction of the Government of Ecuador.
