The helicopter’s fuel system was not inspected by the NTSB or the FAA because the helicopter was in the process of being disassembled prior to the NTSB or FAA being notified of the accident.

Probable Cause and Findings

A total loss of engine power due to fuel exhaustion as a result of the pilot's failure to ensure that adequate fuel was aboard for the flight.
