Factual Information

On June 7, 2013, at 1400 coordinated universal time (UTC), a Piper PA-34-200T, Brazilian registration PT-MMX, was substantially damaged during a ditching into the Tarauacá River following an unspecified malfunction of the airplane’s right engine after takeoff. The flight was operated by W. da S. Gomes ME, and originated from Eirunepé, Brazil. Of the six passengers on board, five sustained minor injuries, and one was fatally injured. The pilot sustained serious injuries. This investigation is under the jurisdiction of the government of Brazil.
